Located just steps from Main Street, the Southlake Academic Family Health Team’s primary clinic serves as the central hub for our services. As the main teaching site for resident physicians—through our partnership with the University of Toronto’s Department of Family and Community Medicine—the clinic supports both learning and high‑quality patient care.
Our interdisciplinary team includes family physicians, resident physicians, nurses, nurse practitioners, social workers, a pharmacist, a dietitian, and a case manager, working together to provide comprehensive primary care to the community.

Clinic Information:
In partnership with the Northern York South Simcoe Ontario Health Team, the Southlake Academic Family Health Team is opening a new clinic to serve local residents in the community without a family doctor or healthcare provider.
The Interprofessional Primary Care Team clinic is now open and booking appointments. Our Family Doctors and Nurse Practitioners provide comprehensive care including detailed assessments, diagnosis, and treatment for a variety of health conditions.
