Our Mission
The Southlake Academic Family Health Team at Southlake is committed to community enrichment by delivering outstanding health care and education to our patients and learners.
The Southlake Academic Family Health Team (SAFHT) was established in 2009 and has been serving the Newmarket area for over 10 years now. We have long been established as a leader of primary care in the community and we pride ourselves on being at the forefront of innovative clinical practice. What is a Family Health Team?
A Family Health Team is an approach to primary health care that brings together different health care providers to co-ordinate the highest possible quality of care for you – the patient.
Although your family physician will be your main source for your health care, a Family Health Team is designed to give primary health care providers support from other complimentary professionals. You will have access to Nurse Practitioners, Registered Nurses, Registered Dietitians, Social Workers, and other health care professionals who work collaboratively, each utilizing their experience and skills so that you receive the very best care, when you need it, as close to close as possible. If the need is there, we have the right person at the right time!
Family Medicine Teaching Unit (FMTU)
Our Family Health Team is in partnership with the University of Toronto and the Family Medicine Teaching Unit (FMTU). Established in 2009 in partnership with the Department of Family and Community Medicine at the University of Toronto, Southlake's Family Medicine Teaching Unit enrolls 9 family medicine physician trainees (seven Canadian and two international medical graduates) every year for a two-year program.
