Hospital Discharge Care Coordination

As part of our Family Health Team’s Quality Improvement Plan, we have taken on the initiative to provide all our patients a follow-up and care coordination plan within 7 days post hospital discharge. Our team has collaborated with Southlake Regional Health Centre, CCAC and even has a Case Manager on site to follow-up with our patients and can provide community resources. 

Requirements: must be an enrolled patient at the FHT
